Frequently Asked Questions
How do I get started as a new patient?
Getting started is simple. You can submit the new patient form, call, text, or email our office. Once we hear from you, our team will reach out to schedule your first visit and go over next steps.
Does submitting the form schedule my appointment?
No. Submitting the form does not confirm an appointment. A member of our team will contact you after submission to finalize your appointment details.
Can I fill out the form for my child or someone else?
Yes. The form can be completed for yourself, your child, a partner, a family member, or a friend. Just select the appropriate option so we know who the appointment is for.
Do I need to prepare anything before my first visit?
You can save yourself time by printing the health history form and filling it out in the comfort of your home and then bringing it to your first appointment.
What happens after I submit the new patient form?
After submission, we’ll contact you using your preferred method (call, text, or email) to schedule your visit and answer any questions.
What information should I include in the comment section?
You’re welcome to share any non-medical details that may be helpful, such as scheduling preferences or general questions. Please do not include any protected health information (PHI) in the form.
What is the fastest way to schedule an appointment?
Calling or texting our office is the fastest way to schedule. If you prefer, you can also email us or use the online form and we’ll follow up promptly.
What if it’s been a long time since my last visit?
That’s completely fine. If it’s been a while, we recommend contacting us to schedule a visit so we can review your current status and help you get back on track comfortably.
